Proof of a physical examination dated within the preceding 12
months, if entering a Florida school for the first time.
(Must
have at time of registration, no waivers)
Physical exam form (DH-3040) COMPLETED and
SIGNED by a licensed physician (Medical Doctor, Doctor of Osteopathy, Doctor of Chiropractic),
a registered Nurse Practitioner or a Physicians Assistant. (A Physicians Assistant must be licensed
specifically in the state of Florida).
The exam must be within the 12 months prior to your child enrolling in a
Florida school, and must have a review of components (complete body systems review).
Children are not admitted to school without this
physical exam form. Contact your medical provider (doctor/clinic)
for a school physical exam (

For students coming from out of state, other forms
will be accepted if it includes a complete review of body systems.
