Lincoln will be holding a Flu Vaccine Clinic on 11/10/2022

If you would like your student to receive this vaccine, please sign

and return the "Flu Vaccine Consent" form no later than November, 1st 2022. 

If you would like more information regarding the flu shot, feel free to

contact the clinic at ext 26002.

If you would NOT like your child to participate, simply do not return the form.
