Our goal at the Ballard Elementary School clinic is to provide the best possible care for your child in the event of illness or injury while at school, and to promote good health and prevention of disease. It is important for us to maintain open communication with you to achieve this goal. Students may need to take prescription medications while at school.
In addition, many conditions can be treated at school with over-the-counter (OTC) medications.
With proper provider prescription/physician orders, authorization, documentation and signatures, arrangements can be made to administer prescription and/or OTC medication to students. This may prevent loss of time out of school.
To have Prescription or OTC medication administered in school:
- Complete the Medication Authorization Form.
- Parent/guardian brings the Prescription/OTC medication to school clinic.
- Clinic staff will process order and verify medication amount with parent.
- Prescription medications must be properly labeled and correspond to the Medication Authorization Form.
- OTC medications must be sealed and not expired.
- Parents must provide all medication, equipment, and supplies as needed at school.
- Unused medications will be returned or destroyed per policy.

The welfare of your child is our primary concern. Completing an Annual Student Medical/Health Information Form/health card provides us with valuable information about your child's emergency contacts, physician/provider office, medical problems/conditions/medications. All parents/guardians must keep the information on their student’s Annual Student Medical/Health Information Form updated throughout the school year as needed.
Please ensure that emergency contact information is kept current and that the school is notified of any changes.
