All forms must be submitted electronically for PreK- 5th grade students. If you do not have internet access, a kiosk is available on campus.

Forms required to be submitted for 24-25 enrollment using the district parent portal for grades Pre-Kindergarten - 5th grade:

1) Birth Certificate

2) FLORIDA Certificate of Immunizations.  (DOH680 form)

3) Physical Exam Certificate (dated within the last 12 months)

4) Proof of Residency minimum of three (Electric, Water or Gas bill, Mortgage Statement, or Lease agreement)

5) Copy of Social Security Card or Social Security Form
