Clinic News and Forms

Dear Parents or Guardian:
During the first week of school your student will be given a Health/Emergency Contact Card and a Consent for Health Services. These need to be filled out, signed by you, and returned to school as soon as possible. In the event of an emergency, every attempt will be made to contact you. If you are contacted to pick up your child from the clinic, you have a window of 30 minutes per our district policy. A student sent home from the clinic with a temperature of 100.0 or higher, must be kept home until fever free without fever reducing medications for 24 hours. We provide care to students in the event of illness or injury while at school. Our priority is to keep students safe and healthy at school. Our goal is to facilitate full participation in class as possible.

MEDICATION GUIDELINES:
For students needing prescription medication or over-the-counter medication during school hours, PMD orders are required. No medication may be sent to the school via backpack with your student. No student may carry any medication on campus, including Cough drops, Benadryl creams, Chapstick, etc. without PMD orders (Medication Authorization Form). Students may Self Carry Asthma Inhaler, Epi-pen, Pancreatic Enzymes and/or Insulin/Diabetic Supplies with a “Contract to Carry Form” completed and signed by the parent or guardian.

MEDICATION AUTHORIZATION FORM:
The “Medication Authorization Form” is to be filled out by the student's PMD. Each medication needs to be filled out on a separate form. Prescription medication and over the counter medication must have the form filled out. The bottom of the form must be signed and dated by the PMD and Parent/Guardian. Parents/guardian must bring each medication in its original pharmacy container, with the completed “Medication Authorization Form.” The medication gets signed in the clinic by the parent/guardian and clinic staff. No medication will be administered at school unless the medication guidelines are followed.

IMMUNIZATIONS:
Please verify with your child's PMD, that their immunizations are up to date as required by Florida Statutes and Florida Administrative Codes for admittance in school. All students are required to have a current completed Florida Immunization Form DH 680 which is obtained from your student’s PMD office or the Health Department. All students are also required to have a physical exam form completed by their PMD. If you have any questions or concerns, please call the clinic. Florida Certification of Immunization: Students entering middle school must be up to date on all immunizations. Tetanus, diphtheria, & acellular pertussis (Tdap) is required for entry into 7th grade. These immunizations are mandatory for students to attend a Florida School and need to be entered by the student’s physician or the health department on a Florida Immunization Form DH 680.
