School Immunizations

Immunizations Requirements for Incoming Students
Florida Statute 1003.22 requires all students entering and attending school to present a Florida Certification of Immunization form (DH 680) or Religious Exemption form (DH 681) to the school. You can get these forms from your health care provider or the Florida Department of Health in Manatee County’s Immunizations clinic. No-cost immunizations are available to eligible children through the federally funded Vaccines for Children program at the Florida Department of Health in Manatee County.
Physical Exam

All students entering a Florida public school must have a physical exam. The Department of Health (DH3040) is the preferred form, but a similar form from the doctor’s office (Florida or out-of-state) may be used if it contains a complete review of body systems.
Where can my student get immunizations or a physical exam?

Out-of-State Immunizations must be transferred to a Florida 680 Form. This can only be done at the Manatee County Health Department.
