Purposes for the Collection of Social Security Numbers

    State law requires an agency to notify individual of the purpose(s) for which social security numbers are collected by the agency.
    Social security numbers (SSNs) are collected by the School Board of Manatee County, FL for the following purposes: 

    1. The Internal Revenue Service and Social Security Administration require SSNs on Form W-4. This form is used to determine how much Federal withholding tax is to be collected and Federal Insurance Contribution Act (FICA) tax on wages paid and later reported in a W-2 Wage and Tax Statement.

    2. The Internal Revenue Service requires a Taxpayer Identification Number on Form W-9, which could be either a SSN or an Employer Identification number.  SSNs or Employer Identification numbers are also used to generate a 1099 Miscellaneous Income Statement based on expenditures processed through accounts payable. Vendors with SSNs are captured in the Vendor Application process.

    3. The Federal Bureau of Investigation and the Florida Department of Law Enforcement uses SSNs as identifiers for processing fingerprints for the required criminal history records checks.

    4. To comply with the National School Lunch Act parents are requested to provide SSNs on the application for free or reduced-price meals. The SSNs are used in the verification process to determine a family's eligibility for their child(ren) for free or reduced-price meals.

    5. The SSNs for employees and dependents are required for enrollment in health insurance, life insurance, and other miscellaneous insurances.

    6. The Florida Department of Education uses SSNs as a standardized identification number for the required reporting of yearly certification and training information.

    7. The Florida Division of Retirement requires SSNs in order to report earnings used to document creditable years of service in the Florida Retirement System.

    8. The human resources/finance software program, PeopleSoft and JD Edwards, requires the use of SSNs as the primary personal identification of employees for wages, leaves, and payroll deductions.

    9. Pursuant to State laws, SSNs are used by the Florida Department of Education as a standardized identification number to track students from year to year and when they move from one school or county to another.  SSNs are used for students in grades 10 through 12 as identifiers for colleges and scholarship programs such as Bright Futures. For students in grades Pre-Kindergarten through 12, SSNs are used as identifiers for enrollment and attendance, funding reports (such as FTE), tracking of achievement gains, and standardized testing. SSNs are included in all Florida Department of Education required reporting.

    10. For adult students and approved GED Exit Option students taking the GED exam for graduation purposes, SSNs are used by the Florida Department of Education as a standardized identification number to track students.

    11. The State Department of Licenses requires the use of SSNs to document that students have passed the written test and completed the Drinking and Driving course requirement for their Restricted Driver's License.

    12. Florida Education Foundation, Inc. collects student SSNs when purchasing savings bonds on behalf of students.                      

    NOTE:  Pursuant to Florida Statutes, the SSNs of all current and former employers held by the District are confidential and exempt from the provisions regarding inspection and copying of public records pursuant to Section 119.07(1) of the Florida Statutes and Section 24(a), Article I of the State Constitution. As confidential and exempt information, SSNs are not subject to public inspection or copying.
